What Your Doctor and Your Herbalist Don't Know About Each Other — And Why That Could Hurt You
June is Herbal and Prescription Interaction Awareness Month, and I want to use this space to have a conversation that does not happen nearly enough in either conventional medical offices or holistic health circles: the conversation about what happens when the two worlds collide inside the same human body without anyone in either world knowing the other is involved.
Let me be clear about something before we go further. This article is not an argument against herbal medicine. Herbal and botanical medicine represents some of the oldest and most validated healing tradition on earth, and many of the pharmaceutical drugs in use today are derived directly from plant compounds. This is also not an argument against prescription medication, which saves lives and manages conditions that would otherwise be debilitating or fatal.
This is an argument for transparency, for informed decision-making, and for understanding a category of risk that is currently causing real harm to real people in significant numbers.

There are nearly 1,500 documented interactions between prescription drugs, herbal medicines, and dietary supplements. In 2024 alone, pharmacists at one major pharmacy services organization intervened in over 7,900 instances of potentially serious drug interactions involving supplements and medications. Those are the ones that got caught. The ones that did not get caught are harder to count, but they show up as unexplained side effects, medications that seem to stop working, bleeding events, cardiac arrhythmias, and hospitalizations that nobody connects to the bottle of supplements sitting quietly on the kitchen counter.
Why This Happens: The Biology of Herb-Drug Interaction
To understand why herbs and prescription drugs interact, you need to understand how your body processes both of them. Most drugs and plant compounds are metabolized through a family of liver enzymes called cytochrome P450, or CYP450. These enzymes are responsible for breaking down and clearing substances from your body, and they do not distinguish between a pharmaceutical and an herbal compound. They process whatever arrives.
The problem arises when an herb either speeds up or slows down the activity of these enzymes. If an herb accelerates CYP450 activity, it can cause a prescription drug to be cleared from the body faster than intended, dropping its concentration in the blood below therapeutic levels and rendering it less effective or completely ineffective. If an herb slows CYP450 activity, it can cause a drug to accumulate in the blood at higher levels than prescribed, turning a therapeutic dose into a toxic one.
This is not a theoretical risk. It is a documented, clinically observed phenomenon with well-characterized examples that every person taking both herbs and prescription medications needs to know about.
The Most Dangerous Combinations You Need to Know
St. John's Wort is the most extensively studied and most clinically significant herbal drug interactor known. It is one of the most commonly used supplements in the world, taken by millions of people for mild to moderate depression, anxiety, and mood support. It is also one of the most potent inducers of CYP450 enzymes known, meaning it dramatically accelerates the metabolism of a wide range of prescription drugs. The documented interactions include oral contraceptives, where St. John's Wort can reduce effectiveness enough to allow unintended pregnancy; antiretroviral medications used in HIV treatment, where it can drop drug levels dangerously low; cyclosporine used in organ transplant recipients, where it has caused documented cases of acute organ rejection; warfarin and other blood thinners; antidepressants, where combining it with SSRIs can trigger serotonin syndrome, a potentially life-threatening condition; and chemotherapy agents, where reduced drug levels can meaningfully compromise cancer treatment outcomes.
The tragedy of St. John's Wort interactions is that the people most likely to take it, those managing depression or anxiety who are also on antidepressants or seeking a natural alternative, are also the people most likely to be simultaneously on other medications that interact with it dangerously.
Ginkgo biloba, taken widely for memory support and cognitive function, has well-documented anticoagulant properties that become clinically significant when combined with blood thinners like warfarin, aspirin, or other antiplatelet medications. The combination can dramatically increase bleeding risk, including the risk of dangerous internal bleeding and hemorrhagic stroke. In the example cited at the beginning of this article, a patient on warfarin beginning ginkgo supplementation is a pharmacist intervention waiting to happen.
Garlic in supplemental doses, not culinary use, has meaningful antiplatelet and anticoagulant effects that interact with blood-thinning medications along the same mechanism.
Echinacea, taken commonly for immune support, can affect the metabolism of certain drugs processed through the CYP450 system including some chemotherapy agents.
Valerian and kava, both used for anxiety and sleep, can amplify the sedative effects of benzodiazepines, sleep medications, and any drug with central nervous system depressant activity to a degree that becomes dangerous.
Grapefruit juice, not technically an herb but operating through the same CYP450 inhibition mechanism, famously interacts with over 85 medications including statins, blood pressure medications, and immunosuppressants in ways that can either amplify or reduce drug effects significantly.
Herbs that mimic insulin activity, including bitter melon and fenugreek, can dangerously amplify the blood sugar-lowering effects of diabetes medications, potentially causing hypoglycemia that can become a medical emergency.
Astragalus, a popular immune tonic in integrative medicine, has been shown to inhibit certain CYP450 enzymes in ways that can affect the metabolism of multiple drug categories.
The Problem That Nobody Is Solving
Here is the systemic failure at the heart of this issue: your prescribing physician almost certainly does not know what supplements you are taking, and your holistic practitioner almost certainly does not have a complete list of your prescription medications. Neither is intentionally negligent.
The healthcare system simply has no reliable mechanism for bridging these two worlds, and the assumption underlying both is that the other world's interventions are not significant enough to disclose.
A 2024 study found that a substantial proportion of patients do not disclose their supplement use to their physicians, often because they assume supplements are safe and natural and therefore not worth mentioning, or because they expect their physician to dismiss or discourage herbal use and they want to avoid the conversation. Physicians, for their part, frequently do not ask about supplements in the detail required to catch interactions, either because time constraints make thorough medication reconciliation difficult or because their training in herbal pharmacology is limited.
The result is a gap in clinical oversight that is not small or theoretical. It is contributing to adverse events that range from annoying, when a medication is working less effectively than it should, to catastrophic, when a combination triggers a bleeding event, organ rejection, serotonin syndrome, or cardiac arrhythmia.
What You Should Do Right Now
Create a complete list of everything you take, every prescription medication, every over-the-counter drug, every supplement, every herbal product, every vitamin, and every mineral, and share that list with every healthcare provider involved in your care. This includes your prescribing physician, your pharmacist, your naturopath, your holistic health coach, and anyone else making recommendations that affect what you put in your body.
Your pharmacist is one of the most underutilized resources available to you on this specific issue. Pharmacists are trained in drug interaction detection and most pharmacy software systems flag potential herb-drug interactions when all substances are entered into the system. Making your pharmacist aware of your complete supplement regimen, not just your prescription list, is one of the simplest and most effective safety steps available.
If you are taking any of the following medication categories, the bar for disclosing all herbal use to your physician should be considered non-negotiable: blood thinners including warfarin, heparin, and newer anticoagulants; immunosuppressants including medications taken after organ transplant; HIV antiretroviral medications; chemotherapy agents; diabetes medications including insulin; antidepressants; antiseizure medications; thyroid medications; and any cardiac medication including digoxin.
Never start a new herbal supplement without cross-checking it against your current medication list.

The integration of herbal and conventional medicine is not just possible. For many people it is the most comprehensive path to health available. But integration requires communication. The two worlds have to know about each other to work together safely. That conversation starts with you.
